Join related rows on npi.
U.S. Dentists Database
Join providers to practice_locations and provider_taxonomies on the text npi field. Keep identifiers and postal codes as text. Dates use YYYY-MM-DD. CSV blanks correspond to JSON null and SQLite NULL.
